To meet the needs of persons in Smith County living in their home, the program Smith Home Health Care was created. Through our Medicare certified home health program we create a comprehensive program for your home health care needs. Whether you’re returning from a hospital, nursing facility or rehabilitation stay, or in need of occasional help – we’re here for you.
Our specially-trained home health teams include nurses (RNs and LPNs), physical and occupational therapists, certified home health aides (CNAs) and social workers who work with your current doctors. Your team provides care focused on recovery and independence in the comfort of your own home.
Am I eligible for Home Health Services?
Yes, if you are homebound and need medical care, you are eligible for home health. Being homebound means it is hard for you to leave your home, and you typically cannot do so without the help of another person or medical equipment. Medicare, Medicaid and many private health insurance plans pay for Home Health Services. Private pay options are also available. The care team will explain your coverage before your admission. In many cases, if you are over age 65, enrolled with Medicare and have a medical diagnosis (or multiple diagnoses) of which you are declining or have new orders or meds for, Medicare will pay for 100% of skilled nursing, PT and OT if needed. To provide our home health services, we have partnered with the Lincoln Home Health Care Services in Lincoln Kansas. Our local staff person will be Amy Timmons a Registered Nurse for over 25 years, with several years of home health care experience. Amy is a lifelong resident of the area and her family resides in Smith County. We look forward to serving you!
